What We Do:


Transition Planning: We help seniors navigate the maze of healthcare and housing options to ensure a “goodness of fit” between the individual’s needs and available resources (legal, financial etc.)


Medication Therapy Management Services: We conduct initial and monthly medication management services for seniors living at home or in assisted living facilities (includes assistance with selection of Medicare D Plan)


Intellivisit™: We conduct in-home or bedside “Professional family” visits to assess quality of life and  quality of care delivery


Intellivist Plus™: During the on-site visit, we use voice and video technology to link the senior with out of town (or country) family by real-time, “virtual” visits.


Consumer Education Services: Team members present education offerings for community groups, medical office staff, long term care and acute care facilities, professional organizations and academic programs. We also provide referrals to professional experts in other service areas such as elder law, financial planning etc. 


Who We Serve:

In addition to seniors and their loved ones, DearWatch clients include trust officers, attorneys, hospital discharge planners, financial managers, primary care providers, in-home service providers and others providing services to senior clientele. Corporate quality audits and consultation services are available.
